[2019 Child Travel Consent Document] Parental Travel Consent Form, Family Relation Certificate

Today, traveling abroad isn’t difficult anymore and a lot of parents are traveling with their children.

 

However, you should be reminded that if you are going on a family trip with minors, you should prepare the following documents;

Family Relation Certificate, Parental Travel Consent Form (Affidavit of Support)

And have them notarized.

Why should I carry a Parental Travel Consent Form?

There are two kinds of Child Travel Consent Documents


Parental Travel Consent Form & Family Relation Certificate.
